Аннотация
Введение
У пациентов с остеоартритом коленного сустава (КОА) наблюдаются изменения в антиципаторной постуральной корректировке (АПК) во время начала ходьбы. Программы физических упражнений настоятельно рекомендуются для лечения остеоартрита коленного сустава, но их влияние на антиципаторную постуральную корректировку не изучалось. Целью данного исследования было сравнить влияние силовых и аэробных упражнений на антиципаторную постуральную корректировку.
Методы
В этом двойном слепом рандомизированном контролируемом исследовании приняли участие 64 пациента с остеоартритом коленного сустава, которые в течение восьми недель проходили интервенцию. Участники были случайным образом распределены в группы: контролируемую (n = 20), силовых упражнений (n = 20) и аэробных упражнений (n = 24). Первичными результатами были предвосхищающие постуральные изменения при начале походки. Вторичными результатами были интенсивность боли, измеренная по числовой шкале боли, физическая функция, оцененная по индексу остеоартрита Университетов Западного Онтарио и Макмастера, и изометрический тест на мышечную силу.
Результаты
После вмешательства в контрольной группе наблюдалась лучшая физическая функция, в то время как в группе с резистивными упражнениями наблюдалось меньшее смещение центра давления кзади и в латеральной плоскости (p = 0,031). Кроме того, в группах с резистивными и аэробными упражнениями наблюдалась меньшая интенсивность боли, большая мышечная сила и лучшая физическая функция.
Выводы
Вмешательства, основанные на силовых и аэробных упражнениях, полезны для контроля интенсивности боли, увеличения мышечной силы и, в сочетании с образовательной программой, улучшения физической функции. Однако эти вмешательства не могут изменить компенсаторные паттерны АПА, используемые людьми с KOA, и у них все еще может быть риск падения.
